Complete an initial comprehensive assessment of your personal situation, including your child’s medical history and social factors that may be affecting their overall health.​​​

Complete a full medication review, including all medications, vitamins, supplements, and over-the-counter drugs, looking for drug interactions and side effects.​

Review your child’s medical records and test reports, providing education about the diagnoses and results in order for you to make future informed decisions.​

Research and provide information regarding your child’s diagnosis and treatment within acute care settings (hospital), allowing you to better understand and make informed decisions regarding your child’s care.​

Advocate on your child’s behalf in the hospital setting.​

Assist with preparing appropriate questions and concerns for your child’s healthcare team.

Facilitate communication with healthcare team and family members, speaking on your behalf as requested.​

Assist with coordination of medical services, such as follow-up appointments, obtaining medications, etc.​

Assist with planning for anticipated future health care needs, such as medical devices or end-of-life care planning.​

Ensure records, labs, and tests are sent and received by all appropriate providers.​

Facilitate communication between your child’s medical professionals.​

Provide information and recommendations for referrals (as applicable) to community resources for caregivers and/or your child, such as daycares for medically fragile children, therapies, licensed counselors, play therapists, or child life specialists.​

Attend medical appointments and/or communicate with healthcare team on your behalf.

Assist with arranging transportation to and from medical appointments.​

Educate you on how to become your own health advocate.​

Provide emotional support during your child’s acute medical events.
